Rheumatoid arthritis is an autoimmune disease that affects joints bilaterally. The associated increase in synovial fluid it causes may be seen on sonograms as bilateral Bakers (popliteal) cysts, which may become large with the possibility of rupture. In this case, incidental findings from a bilateral deep vein study revealed a large infected popliteal cyst in the right leg and a complex lobulated one in the left leg.
